Required documents

1. Application form
2. Diploma of completing secondary education authorizing the holder to apply for admission to the University (with an Apostille). In the case of taking the exam this year - a certificate from the school confirming that the candidate attends the last year of secondary school and will take the exam in the current school year.
3. Transcript of grades from the secondary school (with an Apostille)
4. Copy of a passport (scanned page with personal data)
5. Proof of payment of the non-refundable reservation fee of 200 EUR
6. Statement by parents or legal guardians of a minor (Download the document)
7. After receiving the letter of offer and paying for the course, please send by e-mail the proof of transfer.
8. An insurance policy for health services and civil liability/accidents (OC/NW) is also required.
   Note: Please send a scan of the policy by e-mail to diana.gorska-lewinska@uwr.edu.pl after obtaining a visa and notify us about the day of your arrival in Wrocław.
